About
Dr. Tamir Weinberg, M.D. is a Retina Specialist and Vitreoretinal and Cataract Surgeon at Kaplan Medical Center, affiliated to the Hebrew university, Jerusalem Israel, where he works as a senior retina specialist.
As part of his academic appointment at the Hebrew University of Jerusalem, he is involved in training and teaching of medical students and ophthalmology residents, taking active part in local and international clinical trials in the field of medical retina and international conferences.

After graduating his ophthalmology training in Israel, he then completed a fellowship in
the field of Vitreoretinal Surgery at Sydney Eye Hospital, Sydney, Australia. During his
fellowship, Dr. Weinberg performed hundreds of retinal surgeries and complex
cataract surgeries while gaining experience in the management and treatment of
retinal diseases.
In addition to his public hospital work, Dr. Weinberg has been working since 2015 in
Assuta Medical Center in Tel Aviv as a retina consultant and a cataract surgeon for
Maccabi Healthcare services.

SURGERY
Cataract is a natural process of lens aging that causes gradual visual decline. The lens is naturally located within a delicate capsule suspended by fibers (zonules – thin thread-like structures) attached to the walls of the eye. At birth, the natural lens is completely transparent. As one ages, the lens loses its transparency, becoming opaque, hence causing weakened vision, much like trying to look outside through a dirty window.

Cataract formation is a natural process of the lens gradually becoming opaque with age, hence causing weakened vision, much like trying to look outside through a dirty window.
The aim is to replace the opaque lens with an artificial lens. Using microscopic incisions, we open the capsule containing the cloudy lens. A specialized device breaks and removes the natural lens from within the natural capsule while cleaning all remaining parts of the lens inside. Then, an artificial transparent lens made of acrylic material is implanted into the same capsule. This lens remains in the eye for life.
The surgery is mostly performed under local anesthesia using eye drops, typically without the need for hospitalization. The average duration of the surgery is about 15 minutes (depending on the degree of cataracts). After receiving detailed instructions, the patient is discharged home.
The surgery is performed in one eye at a time, and it's not customary in Israel to perform the surgery on both eyes simultaneously. The success rate of cataract surgery is over 99% with relatively low complication rates. Before the procedure, the patient will receive a detailed explanation and a consent form to review at home, specifying all possible complications and risks
The implanted lens, typically made of transparent acrylic material, comes in various types.
- Monofocal: most common. They correct vision for either distance or near vision.
- Multifocal: offer different focal points, correcting vision for both distance and near.
- Toric: correct astigmatism. This lens is available for both in monofocal and multifocal lenses.
Each type of lens serves a specific purpose in correcting vision, and the choice depends on individual needs and indications provided by the cataract surgeon.
FLACS is a more advanced method compared to traditional cataract surgery. In this approach, an additional step is added at the beginning of the surgery, performed by the laser device itself. Depending on the surgeon's choice, the laser can create precise entry points into the eye, making incisions in the cornea to reduce astigmatism, cutting the anterior capsule of the lens capsule, and breaking down the lens into smaller pieces before the surgery.
After performing these steps, the surgeon continues the surgery manually, completing the remaining stages of the procedure.
Laser cataract surgery is considered safe with very low complication rates, but it may not necessarily be suitable for every patient since there are certain conditions that may not allow the use of this technique.
After the surgery, the patient is released to return home without the need for hospitalization. A transparent plastic eye shield, attached with tape, is placed over the eye to protect it. Typically, the shield remains in place until the morning after the surgery, after which it is only required to be worn during sleep for several days.
Additionally, the patient needs to use eye drops for the operated eye: antibiotic drops and steroid drops, and sometimes non-steroidal anti-inflammatory drops (NSAIDs) in certain cases. Improvement in vision sharpness begins in the first few days following the surgery, but the initial vision check is usually conducted during the second postoperative visit.
Secondary cataract, also known as posterior capsular opacification (PCO), occurs when the natural capsule of the eye, into which the artificial lens has been implanted, becomes cloudy over time, leading to a decline in vision. The treatment for this condition involves a laser procedure called YAG laser capsulotomy. This procedure aims to clear the posterior part of the capsule that obstructs the central visual axis.
The treatment is typically done in an outpatient setting under local anesthesia with eye drops and lasts for a few minutes, with minimal complications. After the procedure, there might be floating particles in the field of vision, which are essentially remnants of the capsule that the laser has disrupted. These particles usually get absorbed by the eye over time.
- Foreign body sensation in the eye: This feeling is normal and is a result of the incisions made in the eye during the surgery. These complaints usually diminish within a few days to weeks post-surgery and don't typically have clinical significance.
- Tearing: This may result from dryness or irritation. This complaint also tends to diminish with time but can be alleviated by using artificial tears drops in the initial period after the surgery.
- Temporary mild vision blur (uncommon): It can result from the effects of pupil-dilating drops given before the surgery, which can take up to a few days for the pupil to return to its normal size. Additionally, fluids may accumulate in the cornea (a condition called corneal edema) after the surgery, which tends to improve in the early postoperative days. In rare cases, the cornea may remain edematous with fluids, usually due to corneal diseases where the inner layer of the cornea has lower functional potential than a healthy cornea.

Vitrectomy is a delicate surgery aimed to remove the vitreous cavity of the eye through tiny incisions. This allows direct treatment of different pathologies such as retinal detachment, removal of scar tissue/membranes, treating macular hole, clearing hemorrhage (blood) from the eye and more.
This procedure my accelerate the development of cataracts. Therefore, a combined surgery for both vitrectomy and cataract removal can be performed.
After the operation, the eye cavity can be filled with fluid/air/gas/silicone oil according to the pathology being treated.
This surgery can be performed under local anesthesia around the eye or general anesthesia. It usually does not require hospitalization, and the recovery is relatively easy and fast.
